Application Process

The following forms are part of the process for registering for therapeutic riding at Seven Gables and are included in this material. The forms must be re-submitted annually:

• Physician’s Medical Release– Rider’s Medical History Form
• Participant Application Form (to be completed by parent or guardian where appropriate)
• Waiver and Release (Minor) or Waiver (Adult)
• Authorization for Medical Treatment Form

Once these forms are completed, the individual can be scheduled for an assessment/evaluation appointment. A minimum of four staff persons is required for this evaluation. The charge for this appointment is $55. If therapeutic riding seems appropriate, goals will be discussed and lesson plans developed.

Special Forms

In the case of Down Syndrome, a Physician Referral Form for Down Syndrome verifying the absence of Atlanto-Axial dislocation or subluxation is necessary.

Should a potential client require a release form, a Participant’s Consent for Release of Information Form designates the specific information required.
